Forms for Parents and Carers

We require that parents and carers provide information for the school about your child, including giving consent for specific pastoral or medical care.

These forms are provided to enable you to share with us relevant information about your child with us, or give consent for some aspects of school. Copies of these forms are also available from the school office.
